The Healthy for Life partnership has a fantastic opportunity for an experienced person to oversee and manage the development; implementation and monitoring of a successful Aboriginal program, the Healthy for Life model and its operations for East Gippsland.

You will provide leadership, support and development of staff and services involved in the HFL model through a commitment to continuous quality improvement, change management and capacity building principles.

The position will also provide executive support to the Djillay Ngalu Steering Committee and Executive on all matters related to Healthy for Life (HFL) and Aboriginal Health Promotion and Chronic Care Partnership (AHPACC) funded initiatives.

Background:
The East Gippsland Healthy for Life partnership is a partnership between three (3) Aboriginal controlled health services (East Gippsland Aboriginal Cooperative, Lake Tyers Aboriginal Trust, Moogji Aboriginal Council) and one (1) mainstream primary health care agency (Gippsland Lakes Community Health) operating in East Gippsland, Victoria.

The consortium formed a joint steering committee to oversee the Commonwealth funded Healthy for Life (HFL), which is a three year primary health care improvement program, having and has two main components:
1. 0-2 child and maternal health and
2. chronic disease management

The HFL program aims to improve the:
• availability of child and maternal health care;
• prevention, early detection and management of chronic disease;
• long term health outcomes for Aboriginal and Torres Strait Islander Australians; and
• Aboriginal and Torres Strait Islander health workforce

The Victorian State Government funded Aboriginal Health Promotion and Chronic Care Partnership program similarly focuses on chronic disease and its management and health promotion based early intervention strategies.

The consortium formed a joint steering committee to oversee both projects. The project was named Djillay Ngalu. Total budget of Djillay Ngalu is just over $1,000,000 per annum.
